Please review this checklist before your visit. Arriving with the right documents helps us serve you quickly and efficiently.

Required Documents

Physician's Order / Referral — the form completed by your doctor ordering your exam. Required for all PET-CT scans.
Government-issued Photo ID — driver's license, passport, or state ID.
Current Insurance Card(s) — bring all insurance cards active on the date of your service.
List of Current Medications — including dosage and frequency.

Helpful to Bring

Previous Imaging — CDs, films, or printed reports from prior scans related to your condition.
Relevant Medical Records — recent lab results, surgical reports, or pathology reports your doctor may have referenced.
Referring Physician's Contact Info — name, address, and fax number for sending your results.
A Companion — especially for PET-CT scans, which take 2.5–3 hours. You may drive yourself home after.

Payment & Financial Responsibility

Any applicable deductibles, co-payments, or co-insurance amounts are due at the time of service. We will contact your insurance company to provide an estimate when possible, though the final amount may vary until your claim is fully processed.

Insurance Pre-Authorization

If your insurance company requires a pre-authorization for your procedure, our center will work directly with your referring physician to obtain the authorization before your appointment.
